Library Instruction Request Form

(BSC Faculty)

As Information Literacy is an outcome of the Undergraduate Core Curriculum at Bridgewater State College, the Librarians are looking forward to discovering ways that Information Literacy can be implemented into classes. The Librarians at Maxwell Library can collaborate with faculty members in a number of ways to deliver Library Instruction to students. It is our hope that we will be able to work with faculty in creating assignments that involve research and critical thinking skills. For those familiar with our traditional Bibliographic Instruction sessions, you can still request this service by filling out the Bibliographic Instruction form below. Please also note that we have included some additional options to our request form. For more information or questions please contact Pamela Hayes-Bohanan phayesboh@bridgew.edu, Coordinator for Bibliographic Instruction, or your Department liaison.

Course Name:
Course Number:
Number of Students:
Preferred meeting date & time:
(Please note a librarian will get back to you to confirm a time. We will accommodate as our schedules permit.)

You are not limited to a one-time session. If you think your students would benefit from several shorter sessions, please let us know.

The library has an electronic classroom with thirteen PCs reserved for Library Instruction. It accommodates up to 26 students. You can bring your students to the library, or we can come to your classroom.

Where would you like to have the session?

Are there any particular databases or indexes you would like demonstrated?
(See Electronic Databases page for available databases.)

Will the students be completing an assignment based on the Instruction session?

YesNo

If so, please provide a copy of the assignment to the Reference Department a week prior to the class.

While we strongly recommend the course instructor's presence, it is not required. However, in these cases students must have a Library assignment to complete during the class session. If you need help developing an assignment, please contact the Reference Department at (508) 531-1394 or refdept@bridgew.edu.

Are there any specific keywords, concepts or phrases you would like us to use in demonstrating searches?

<//td>

Would you like to enroll a librarian into your course Blackboard space?

Yes No

If you answer yes, a librarian will be contacting you shortly.

Any other special instructions, or ideas?

Thank you!
